HEDIS Compliance

APEX has developed a HEDIS focus compliance program, developed to help ACOs, Health Plans, Physician Groups and Value Based contract participants.

APEX, recognizing that many patients are “Home Bound” and have a challenge accessing their assigned Medical providers, because of their physical or mental limitations, has established this patient outreach initiative. As participants in organized medicine (Patient Members of Health Plans, ACOs or Physician Groups) they are required to receive a number of HEDIS Clinical Quality Indicators, to measure the performance of that organized Medical entity and the degree of patient compliance with well-established clinical indicators – set by the National Committee on Quality Assurance. APEX has supported these entities and through the following established methodology.

Utilizing APEX’s Home Health structure – the APEX Medical Director receives the requested information from an entity being held accountable for attaining specific patient clinical values (i.e. Diabetics require A1c lab tests). The APEX Medical Director then generates the “medical order” authorizing the APEX appropriate provider – NP / PA, RN, LVN or Healthcare Technician, to call patient, explain that they need to go by the patient’s home to obtain the medical specimen ordered by the physician. APEX works with the requesting entity to send the specimen to the Lab Service they prefer. Once a result is generated, the result is sent to the Physician’s office (Medical Home Physician) and the ordering APEX Medical Director. The Health Plan, or other contracted entity, would then be able to obtain the lab report from the Patient’s Medical Home Physician’s office.

The process would work, in similar fashion, when administrating preventive interventions, such a flu shot, pneumonia vaccine or COVID – 19 vaccines, for all of those patients who are physically limited from leaving their homes.
