APEX has identified Remote Patient Technology, as a key tool for physicians to work with their patients. By daily monitoring the patients’ blood pressure levels or blood glucose levels – the attending physician can both monitor the patient medical condition and evaluate the effectiveness of the pharmacological interventions – as part of the patient’s plan of care.Patients eligible for this program, who have Medicaid and Medicare (MQMB) qualify for this service and who have;

1. Hypertension or high blood pressure concern.
2. Diabetes, or gestational diabetes during pregnancy.
3. Patients going to have transplanted solid organ or those who just completed the transplant organ process.
4. Congestive Heart Failure.

By utilizing the most recent patient monitoring technology and an internet digital platform – APEX is able to communicate in a real time basis – patient results to the referring physician. Abnormally high, dangerous readings are first investigated by APEX Registered Nurse and upon verification – the nurse contacts the physician directly and provides the clinical concern.

This Medicaid sponsored program, does not affect Medicare reimbursement and is focused on two primary diagnosis – Diabetes and Hypertension – for patients who have both Medicare and Medicaid payer sources.

APEX has additionally, created an extensive compliance program – which is based on the Texas Medicaid Healthcare Partnership (TMHP) procedural guideline and the Office of Inspector General’s past identified “shortcoming” of audited providers. APEX facilitates the internal controls, within the physician’s practice to adhere to policy guideline and on a periodic basis, performs office audits – to assure compliance with written State policies.
